Absent/Late Form
Students who are going to be absent should complete the form below to notify the office.
Dance Level Placement Contestation Form
Please complete this form to request to appeal for consideration to trial a dance level other than your current level.
Completing this form does not guarantee a trial class, but will initiate a conversation between your instructor and the head of the dance department regarding next steps for your progression.
Injury Verification Form
Students who sustain an injury, whether during a class/performance or outside of school, must have a regulated medical professional (family physician, nurse practitioner etc.) complete this form as soon as possible. Please submit completed forms to Student Services.
